Un/Subscribe Form

  Instructions:

1. To Subscribe:
   Enter your full name & email address in the form above and click on the [Submit] button. You will shortly receive via email a reply confirming your subscription and telling you about the AAPAE Email List.
2. To Unsubscribe:
   Enter your full name & email address that you used to subscribe to the AAPAE Email List in the form above, select the (unsubscribe) radio button and then click on the [Submit] button.
3. To Reset:
   Click on the [Reset] button to clear fields so you can re-enter again.
